To Whom it may concern:

My name is Matt Fell and I have a medical malpractice complaint against Prairie View Residential Facility of Fayette Iowa in the form of my  overly excessive incarceration sentence without just cause that greatly exceeds original quote to render services in 5 to 6 months timeframe by Prairie View Residential Facility of Fayette Iowa which is unethical and bad which is medical malpractice as over-incarceration causes me harm in the form of I do not want to  be court committed nor reside at Prairie View Residential Facility of Fayette Iowa which is anti-ethical which is both wrong and bad.

I wish to speak out against the injustices committed at Prairie View Residential Facility of Fayette Iowa where medical malpractice is rampant!  There are other medical malpractice complaints included as well.  I want to reside in my own private residence and was originally quoted by the “RHD” and/or “IHH” programs affiliated with Prairie View Residential Facility of Iowa (housing incarceration programs) that I would be allowed to live in my own private residence but they changed their mind (an abuse of power) which is both unethical and wrong.

The “RHD” and/or “IHH” programs justification was a history of substance abuse which can be argued for or against and should have been aware of before they told me I could live in own private residence, very unprofessional which is bad.   in my opinion I do not now nor have ever really had a substance abuse problem this can be argued for or against.  I do not wish to be at Prairie View or court committed!  I am entrapped in the court committal system for no good reason or just cause, projected condemned to the court committal incarceration for life for myself.  Condemned to court committal incarceration constraints thereof sentenced to life (currently) in said court committal incarceration phases thereof as the incarceration comes in so many different forms i.e. assisted living incarceration for future incarceration phases and so on all projected for myself without just cause (you can argue for or against it being justifiable) and when I am allowed to live in my own private residence after the next phase of incarceration the “hab home” where you live in a house with strangers and 24 hour support staff, non-necessary services for in my opinion myself such as “welfare checks” and so on they come in several different forms thus causing future phases of forms of incarceration; for example when I am eventually allowed to reside in my own private residence with forced “services” i.e. “welfare checks” as they  come in several different forms all forced upon myself, all without just cause (this can be argued for or against) which my opinion  is unethical and bad an injustice as there are many injustices  at Prairie View Residential Facility of Fayette Iowa.   I was found to be mentally competent and no psychological diagnosis was issued at case study number one while I was in federal prison in 2013, case study number one completed by Dr. Dwire of FCI Englewood of Littleton Colorado.

In exactly late May of 2017 I was sent to MHI Independence of Independence Iowa from Prairie View without any voicing of concern prior to transfer whatsoever as to my apparent behavior that was of concern.  I was merely called to the office area in exactly late May of 2017 and told must leave for the MHI and could not take anything with me.  There was no voicing of any concern whatsoever as to the behavior of mine that would warrant such a transfer.  I was at MHI of Independence Iowa for about a month and a week I took two psych tests and was told I scored average.  I claim medical malpractice as the transfer to MHI was non-warranted as I am a well behaved individual and there was no voicing of concern whatsoever by Prairie View Residential Facility staff as to my apparent behavior that would give cause for such a transfer.  I claim medical malpractice as the transfer caused me harm in the form of I do not want to be at this facility (Prairie View) or court committed for valid reasons and the transfer added to my overall incarceration time at this facility which is the concept of causing me harm as I do not want to be here and the original quote to render their services in 5 to 6 months has been greatly exceeded almost one year incarceration at Prairie View Residential Facility of Fayette Iowa without just cause, the concept of medical malpractice had been committed which is non-ethical and void of all ethics which is wrong and bad.

 I plea to have my freedom reinstated i.e. to have my court committal dissolved or broken immediately (miscellaneous motions in order to do so filed pending resolution via the Iowa Judicial website via efile filed under case filing number of my court committal).  I have as court committal incarceration comes in several different forms i.e. assisted living, “welfare checks” and so on for when I reach the phase of court committal incarceration where the incarceration comes in the form of “welfare checks” in several different forms when I will eventually be allowed to live in my own private residence with said incarceration services forced upon me without just cause (you can argue for or against this as being justifiable) as I have been wronged by the court committal system which is unethical and bad!  I am not a threat to myself or others nor have I ever been and have insight into my apparent mental illness and am not a violent person all grounds to break or dissolve a court committal (miscellaneous motion requests filed pending resolution in order to do so filed under Iowa judicial website via efile feature) which is what I wish to have done.  I wish to have my court committal broken or dissolved (request pending via Iowa Judicial website filed under court committal case filing number:  MHMH016293 for Matthew David Fell of the state of Iowa).   

Court committal incarceration comes in several different forms and as I have several projected future phases of incarceration projected for myself for life.   I am  projected sentenced to life in court committal incarceration without just cause of good reason (“hab home” incarceration housing the next projected phase of incarceration than the non-necessary “services incarceration” i.e. welfare checks as they come in several forms thus my privacy will be infringed upon) to my understanding I have been condemned to life time court committal incarceration in all of the incarcerations phases as there are several different forms of incarceration without just cause for the incarceration phases, which is wrong and unethical projected for myself which is unethical and wrong without just cause as I want my freedom back and do not want to be court committed.  I am not now nor have I ever been a threat to myself or other and have insight into my apparent mental illness to my understanding said statements are grounds to have a court committal broken or dissolved.

 

 A medical malpractice complaint in the form of overly-excessive incarceration time at this facility (Prairie View Residential Facility of Fayette Iowa) that greatly exceeds original quote to render said services without just cause (in my opinion, this can be argued for or against) to increase overall incarceration time as has been committed medical malpractice act their of i.e. I will have been at Prairie View Residential Facility of Fayette Iowa for one year in mid-February of 2018.  My incarceration has greatly exceeded the original quote of 5 to 6 months for Prairie View to render their “services” as I will have been at Prairie View Residential Facility of Fayette Iowa for one year in February of 2018.  Acts of medical malpractice their of committed by Prairie View Residential Facility of Fayette Iowa which is wrong and exceeds original quote of 5 to 6 months to render their “services” (thus causing me harm in the form of excessive incarceration time thus adding to my overall more constrictive  incarceration time, the concept of causing one harm is medical malpractice which has been committed in the form of I do not want to be court committed or incarcerated at Prairie View Residential Facility of Fayette Iowa thus causing me harm the concept of medical malpractice due to my loss of freedom in the court committal system as I am sentenced to the non-necessary “services” of incarceration  projected for life!   I have a lifetime sentence in the court committal system to which I wish to have broken (court committal, request pending in order to break or dissolve my current court committal filed via Iowa Judicial website efile, filed under court committal case filing number pending resolution to miscellaneous motion requests to break or dissolve my current mental health commitment). 

 I believe at the core Prairie View Residential Facility of Fayette Iowa operates for a just cause there mission statement is admirable but not in my case as I want all of my freedom back.  I want to be free from my court committal and the forced incarceration “services” that it comes with.  There are  other people at this facility  whom have been condemned to overly excessive incarceration sentences in my opinion without just cause at this facility; this facility needs more external non-biased oversight into the program planning for all individuals to prevent as in this case non-justifiable (you can argue for or against it) over-incarceration without just cause that exact original generic quote (given to all individuals here) of 5 to 6 months to dispense Prairie View Residential Facility of Fayette Iowa’s “services” has been greatly exceeded for myself without just cause which is immoral and unethical very unjust, a miscarriage of justice without proper or any justification as I have no rights but the one’s this facility ascribes to me which is immoral and void of all ethics! 

 I have been a well behaved individual at this facility and actively participated in all required courses and am in the final course which I attend and will never end until the day I discharge to the next phase of lifetime incarceration a.k.a. the “hab  home”.  The excessive incarceration time at Prairie View Residential Facility of Fayette Iowa exceeding the original 5 to 6 month quote to render their services is an unethical abuse of power without enough impartial oversight by external agencies as to protect the patients’ rights and should not always rely solely on the individuals “team” as there is no input from the individual as to what they want for services that become services that are forced upon one such as myself.  I have no input into my program planning an unethical injustice as to what happens to me at and beyond Prairie View Residential Faculty of Fayette Iowa which is void of all ethics an abuse of the court committal system which is void of all ethics which is bad.  The concept of causing one harm in the form of overly excessive incarceration exceeding original quote of 5 to 6 months (generic quote given to all whom enter this facility to my understanding) to render their services is the act of committing medical malpractice as I have been condemned to this facility for too long and condemned to the court committal system for life!  I do not want to be at this facility or court committed and feel I have valid reasons to break or dissolve my current court committal! I want to return to life beyond the constraints of my court committal incarceration as it comes in several different forms.   I plea to have my court committal broken  or dissolved my non-necessary court committal as I believe it has served its purpose and run its course as I have learned allot from my programming courses at Prairie View Residential Facility of Fayette Iowa and am ultimately better for it.

 I have had two failed appeals to my mental health commitment and a failed placement hearing request recently I was advised I can ask for another placement hearing request once I enter the “hab home” phase of court committal incarceration.  Due to what I believe are inaccuracies in the state brief (appeal to my mental health at the Supreme Court level for the state of Iowa Supreme Court brief No. 17-0031, a failed appeal) and a failed appeal at the local level with a judge Harris of Waterloo Iowa.   I am currently asking for an investigation into the inaccuracies of the state brief that began as a physician’s report with false information in it that became part of the state brief at the time in December of 2016 (physicians report with false information in it) when I wrongfully entered the court committal system at Allan hospital of Waterloo Iowa.  There were major inaccuracies of the physicians report at the time of December of 2016 from Allan hospital of Waterloo Iowa that I am currently contesting resulting in denial of my ADD/ADHD medication and so on as I have a pre-existing ADD/ADHD diagnosis from 2008 which is medical malpractice for physicians report that had false information in it pertaining to myself owning or possessing bombs and/or bomb making material or components of researching their of which is false as I have never do so in my life I can only plea and hope that you believe me as I am not a violent person (request for investigation into physicians report from 2016 with false information in it and false information in state brief pending filed under Iowa Judicial website efile filed under court committal number MHMH016293 for Matthew Fell.   

I was also wrong about many statements that made it that made it into the state brief and there are inaccuracies and misunderstandings in it (state brief) for why I should be court committed.  I have filed the miscellaneous motion for requests for an investigation into physicians report with false information in it that became part of the state brief via Iowa Court Judicial website Efile feature recently pending resolution and filed an email inquiry (per advisement from doctor from Allan hospital of Waterloo Iowa in December of 2016) with the Department of Corrections filed in December of 2017 having received no response as of yet to my email inquiry. As a side note I had a DMV/DOT medical report that needed to be filled out Prairie View Residential Facility refuses to fill out the form so I can get my drivers license reinstated which is cruel and wrong for no good reason whatsoever as I had a former doctor fill out the form but I had not been seen by him in the specified time frame per DMV/DOT policy to be valid so the form is capable of being filled out, a reasonable request which is being denied by Prairie View for no good reason whatsoever an unethical injustice that is cruel and bothersome which is bad.

 

I wish to speak out against said injustices, overly-excessive sentences served out at Prairie View Residential Facility of Fayette Iowa where human rights are denied and services i.e. original quote of 5 to 6 months to render services which will be a year in mid-February of 2018 are greatly exceeded.  I am not the only person at this facility (Prairie View Residential Facility of Fayette Iowa) whom this has happened to i.e. serving overly-excessive incarceration sentences in my opinion without just cause which is an unethical injustice!  The resolution I seek is to have more oversight as to what happens at Prairie View Residential Facility of Fayette Iowa  from external sources that seek the individual’s opinion as well to prevent overly-excessive incarceration sentences from happening to people as they have happened to more people than just myself!  I thank you for your time!
