Liberty Online Services Aka Hold Billing Services cramming fraudulent billing Nationwide

I got my Qwest phone bill. It included a $29.95 charge plus taxes for Liberty Online Services. I didn't request this service. In fact, I'm surprised Qwest didn't catch it since I use their online service. I will be contacting Qwest immediately.
