Integrated Rehab put me on a table /bed and turned on a home type electric massager (sold/Sharper Image) Omaha Nebraska
I had back Pain so I got a appointment at Integrated Rehab from an add in Phone book as,My had retired
so on Feb.23/2015 at ? 11:00 filled out papers and was taken to a room put up on a table with a Sharper Image
chair massager turned on for 30:00 Mins. Now on the 24 th of April 2015 I recieve a bill of $226.00
Less what the Ins. co Paid out and my Copay I owe $122.16
