I have been a Discover Card holder for many years.  I used my card to secure a rental car from Hertz while my car was in the body shop for a bumper repair.  I declined all "optional" services that the rental car/truck agencies offer you; specifically the one that said that I would "prepay" for gas.  I had to return the first car rented to me because of a high pitched whining sound coming from the engine. I filled the tank with gas, although I didn't drive it more than 25 miles.  They gave me another car and said they would transfer my paperwork over to the new car. With that said, the original contract was not effected.  However, when I returned the second vehicle, I had filled it up with gas as well before returning it.  Several days later, I was checking my Discover Card statement online and noticed that Hertz had charged me $63.17.  I called Hertz and asked them what that charge was for and they told me that it was for the "prepaid" gas option that I agreed to and also for the "optional" insurance. 

I checked my original contract and both of those items were clearly declined with my initials next to them.  I contacted Discover to dispute the charges and they temporarily took them off.  After a month, they reapplied the charges and told me that I had the burden of proof.  I sent them a copy of the rental contract along with Hertz but that wasn't good enough.  They took Hertz' work for it that I owed them this money.
