This could have been a genuine error on the part of the staff. The lunch and dinner rushes can be especially draining on them... If you've never worked fast food service, there's a system that they should be following where food made at a certain time gets written off and discarded within a set amount of time after it's put on the shelf or in the hot holding trays. It can be easy to confuse the fresh orders for holding with the stuff that's still around immediately after the lunch rush. It *does* happen from time to time. When it comes to microwaves in these places, the microwaves are NOT used to outright "cook" food. They are simply used for a specific short interval to quickly reheat an item (in most cases, the burgers) to an appetizing level. Yes, putting a burger that's past it's "shelf time" will result in the lack of quality you got. One tip is to try to wait at least an hour after a "rush" period if you can. Your chances of getting the quality of food you're used to will be greatly increased. It also seems that you haven't really had a problem like this with your location in the past, so this could be a one off fluke. I suggest mentioning it to the management next time you're in. Can't hurt.
