MasterCard gave me a new credit card because of a theft-identity abuse of the old card.  So I called Affinity to give them the new card number, as this is how they collect the quarterly premiums.  I learned on the phone that because they couldn't collect from the old card, they cancelled the policy.  They apologized for this--anybody can make a mistake--but my problem is that they were unable to fix the mistake.  The policy is due to expire on Jan1, (3 days from now) and to reinstate the policy the company needs a signed form from me.  Fine, but they have no way to email of FedEx the form to me so that I can print, sign, scan, and return it before Jan 1.  Instead, they are mailing it (US Mail; 5-10 business days!), by which time the policy will have lapsed, leaving me 'uncovered' through no fault of my own.  I offered to mail them a letter tooday, with a check for the premium, but they wouldn't even accept that.  They seem to be bound to their forms and to their comppany policies, rather than to seek innovative ways to solve a problem.  Again, my complaint is not that they made a mistake, but rather that they seem unwilling/unable to fix it.
