Complaint Review: Amerigroup Medicare Medicaid - Austin, San Antonio, Covington, Virginia Beach Texas, Kentucky, Virginia

 Typo Correction on today's report. Regarding Texas Medicaid Medicare Amerigroup Anthem. My osteoarthritis of the lumbar spine and my severe advanced marked Cervical Spine Cervical Spondylosis stage 4 DDD disc type on c56c67 was faded out and dismissed, and changed in medical records to a coded false diagnosis: Osteoarthritis of the Cervicothoracic spine, which I do not have. I made typo error Cervical Thoracic spine. Will add that my skeletal spine, sciatic nerves, cervical nerves, rotator cuffs, and shoulder labrum are all formally diagnosed physical medical conditions in past and are degenerative and with proof of and in med records.They gaslight you like you just starting out and have no diagnosis(es) when you tell them every visit your diagnosed medical conditions and note them in previsit form and they all already have all info. Which you just have to do in this outfit. I do not have any physical medical conditions that cause pain and symptoms that are not ALREADY diagnosed. Was taking 2 meds one since 2008 and one since 2012 for the skeletal spine diagnoses and they boldly dismissed them and Dr. C.M. put last few times cervicalradiculopathy M54.12 in place of the real diagnoses. M54.12 is a, pain in the cervical spine in neck that radiates and they do not know what it is, garbage can diagnosis, and can include psychological or stress as a reason. Total balony. Plus add Dr. C.M falsified nonorganic pain malingering cervical spine exam that she never did and put positive for tenderness, which is faking pain and pain in head, and she bogusly put that in my records along with that. Gaslighting. I already have diagnoses long time ago on all conditions and they all physical. I stopped the 2 meds as Dr. Grifter AKA Dr.C.M. sleazily boldly changed what meds for. I use over counter meds and topicals ice, and new infrared heating pads just got instead.

 This grifter ProfessionalConartistPhysician(PCP) ToxicPanderer(TP) gaslighting unscrupulous liar Dr. C.M. Austin, Texas, has caused a disruption for me in clinic am at. How do I get another pcp? Nobody wants to take this insurance. I do not have out of pocket and everybody on benefits through social security is assigned medicaid or medicare and not eligible for indigent clinics. Was on disability for skeletal spine and on elderly indigent health insurance now. In writing told clinic that I do not want to leave clinic, but not going back to Dr.C.M. and reason why. I am noting some previous info for first time readers of this issue, as may be cumbersome to read all past stuff. Have not gone to the clinic as a patient so do not know about quality of as am assigned this ghettofare indigent malevolent adversarial health insurance that discriminates on patients who are not on health insurance through employment or passed employment. Most people sooner or later get on employment related health insurance but I can not, so profile flagged even more. They could not care less what your physical medical diagnoses are, or about facts or the truth. There FAKE NARRATIVE is to make the patient appear lazy and a faker with psychological pain and symptoms all their lives, and that they have never had any physical medical diagnosis(es) their entire life, that would cause them to not be on employment related healthcare insurance. They fabricate symptoms and pain you not have, and lie that you tell them ridiculous things to make you look like a non credible dingbat. Have had some other dirty staff in past and they like to crony up n make like you just a chronic complainer which you are not. Fake narratives and fabrications and aspersions to note that you have anything else but a physical medical condition causing your pain and symptoms is there balony agenda. I was 66 years old when I was at last visit July 20, 2021 and this grifter PCP lines her greedy pockets and lies in my medical record for the health insurance, that she did an out of the blue for no reason, "Cervical Spine Exam" and all it said was *tenderness positive, nothing else, with a note right next to: She said her symptoms started after a fall in her 20's.That is a malingering non organic pain test. No cervical spine exam done and I never fell on my cervical spine or had pain or symptoms in my cervical spine until in my 50's and a 2012 mri she has showing severe advanced marked DDD 2 cervical spine levels cervical spondylosis with central canal and foraminal stenosis showing on and uncovertebral disease that she dismissed. Never complain about pain or symptoms and never been a drug seeker and they do not do treatments and cannot fix or change my diagnoses they are degenerative. Dr. C.M did do a nonorganic weakness malingering exam on my lower extremeties that was normal. But did not see it noted. Hypocrite Dr. C.M. rollin her eyes tongue in cheek phony mindgames. They get there fake narratives by fabricating, deleting, censoring, ignoring, deflecting, prevaricating, and distorting facts. And she said I told her I had osteopenia. She is a dirty lieing greedy doc. She dismissed my medical diagnoses including osteoporosis by lieing and saying I told her I had osteopenia to add to more of my medical facts being dismissed ignored not noted, to make it look like I had no physical "work" restrictions. And I am going to go back to her? Certainly not, even though this duckfare health insurance will pay the next staff off. They want benefits cut so lie and make like I am not credible, and to make like should never have been on indigent benefits. Only had one treatment done whole time on this scam health insurance that was worthy, otherwise has been worthless. Restating some info to be seen here, so handy in this update. The clinic n all Dr. C.M. cronies will cover for her. I also have 2012 last mri with lumbar osteoathritis DJD L5S1 severe advanced marked DDD stage 4 disc with endplates n discogenic endplates changes above and below the discspace & central canal and neuroforaminal stenosis seen on. This was done supine with my knees bent and propped over a wedge that changes spine angle and removes decompression. Also a lumbar tiny nerve root tumor with potential to grow. A 2014 mri showing rotator cuffs tendonosis with dimunitive shoulder labrum with shoulder labrum arthritis. Dr. C.M. has all info and dismissed. Was a crime victim in my 20's and had several multiple falls down stairways with a sacrum injury with objective evidence diagnosed with sensory peripheral neuropathy in 1990's and nerve damage in 80's. My back had no affect on. I was a domestic unpaid worker for 20 years, housecleaning, taking care of elderly family members and kids and a farmhand. Later after I was a single mom. My low back ddd arthritis started in my 40's and showed on first lumbar mri in 1998. I could not get enough credits for health insurance type employment because of physical medical conditions all diagnosed and did things without standing in one spot or took breaks from walking and I had companions unmarried to that I did unpaid domestic work for and if I had married them I would be on spousal benefits today. All my physical medical diagnoses & records given to pcps and med staff. Saw in my records assessed with muscle pain which is a lie &that is psychological pain. A false coded diagnosis osteoathritis of the cervical thoracic spine is in there and I not have that. I have advanced c56c67 DDD cervical spondylosis. Also have bilateral lumbosacral radiculopathy and cervical bilateral radiculopathy secondary to the skeletal spine conditions totally unrelated to the feet and legs sensory neuropathy or any past injuries. The sensory lower extremeties neuropathy is fluffed off in my records due to inadequate health insurance but I noted in records about it. That and everything else is clearly in records with clear copies of mris and Dr. C.M had everything. Also already been there done that on legitimate physical exams and psychological exams in past and have no history of non organic pain or malingering and psychologicals normal. Given Amerigroup Amerivantage temporary cause of covid and in 2023 will be put on Texas Medicare as all 65plus put on in Texas the Texas type medicare off of medicaid. Same type "element"of health insurance though. So Dr.C.M gets her piece of change and I am stuck for a doctor or nurse to go to. They think you are a pariah and nobody will listen to you or believe you and they pass around these fake medical records spreading it around to third parties. They do not acknowledge corrections or complaints, and ignore or change them to something else and no accountability. Think may have to do this so stating, all said my opinions.

I am GollyMolly author, and all stated my only opinions. I am CORRECTING TYPO error from yesterday. Dr. C.M. whom I have seen 4 times at clinic I go to, that put a LIEING, PHONY positive * for tenderness cervical spine exam July 20, 2021 in my medical records(Which is obviously a MALINGERING test in which skin on neck felt n neck touched, that was NOT done, nor was ANY cervical spine exam done by her) doctor C.M. also noted, Assessment oteoaoarthritis of CERVICOTHORACIC spine M47.813.

I accidentally put TYPO error that she put CERVICAL thoracic spine. She put only C7 T1. She edited out all my cervical skeletal spine conditions and all my lumbar lumbosacral spine conditions that are permanent, degenerative, and what I have been taking the meds she prescribed for. (Since 2008 for my lumbar lumbosacral skeletal spine and since 2012 for cervical spine been taking. Last mri's 2012 so worsened since.)

I am not exclusively taking the meds for cervicothoracic mild disc bulge, which of course is only c7-T1 on skeketal spine, that was seen on 2012 cervical thoracic spine mri. Doctor C.M.who is one of payed unscrupulous puppets for AmergroupAmerivantage MedicaidMedicare that is payed off to put in records Baloney stuff.

As I said I have no hx of malingering or pain in head. If noted would be a totally faked hearsay slander libel or bearing false witness by somebody for this OLDY MOLDY ARCHAIC false narrative that is so desired by AmerigroupMedicaid Medicare particularly on indigent or elderly or people with physical disabilities or physical conditions.But can happen to anybody.

Everytime I go to doctors or med staff I say or write in my med condition n the meds for them.I never go in complaining about pain, or saying I am coming in about "pain". Not interested in talking or thinking about pain n already am diagnosed by licensed medical doctors what is CAUSING pain when I have pain. I state my medical conditions not unspecified "pain".

Their archaic narrative, that they sooooo embrace, is noting patient saying they have pain or complaining about it, when they are not doing that, because that equals to them that patient has pain in head and is preoccupied with pain baloney. If they say you not have pain or are doing well that equates to them your medical conditions are better, resolves, cured or disappeared! Baloney Stuff.

Simple minded moldy oldy broken record mindgames. They have deceptive practices including letting you think they acknowledge what you say n write n that you are getting treatment or meds for factual diagnoses that you have.(Not much treatments as am not lookin for drugs n cannot do anything for diagnoses n they not cover experimental treatments that would help only very little.)

l Like every visit have to tell them verbally and also note it on intake form l5s1ddd severe(bone on bone type)lumbar djd cervical spine 2 levels severe ddd spondylosis coming for also spinal stenosis showed on 2012 mri's meds refill for.

GollyMolly update. All stated my opinions. So July 20, 2021, as I noted, Amerigroup Amerivantage MedicaidMedicare Texas, I am in Austin, Texas(also Virginia&Kentucky) paid my pcp Dr. C.M; to lie in my med records, and to Centers for Medicaid Medicare, that she is associated with, that she did a malingering test cervical spine exam on me, that was positive, that she never did!!

She did NO cervical spine exam at all! Never had positive malingering test ever in my whole life, and was 66 at office visit. Dr. C.M did do a feet leg strength malingering test on me that day, Baloney Sandwich, that was negative but who knows if she noted it n what she noted somewhere. Why even go there! I obviously do not want anything to do with this doctor anymore.

This sleaze outfit, will just give me another sleaze type payed off pcp if I try and change, and no place takes this insurance. I need a common medication that is inexpensive that requires a yearly blootest with prescription. Also take prescription antiinflamatory for lumbar spine L5S1 severe advanced bone on bone type DDD with lumbar osteoarthritis and cervical c56 c67 severe DDD and spondylosis with bilateral lumbosacral radiculopathy and bilateral cervical radiculopathy.

Last Mri's both done 2012 and clinic and pcp has all info in full. Take meds few times a month or so spasm med for L5S1. Since am on SSA benefits, required to take mammogram every 2 years, yearly FIT test, and yearly general bloodtest. Would have to pay in full out of pocket as not eligible for indigent clinic since on Texas medicare n medicaid.

The medicaid is temporary in Texas till Covid ends. Had 4 visits with Dr. CM. First one she puts back pain joint pain neck pain.And puts osteoarthritis unspecified complication status vague diagnosis why am taking meds n what my diagnoses are. Also she noted for osteoarthritis cervicalthoracic spine.

The insurance had her edit out my med conditions, and what takin the meds for since that I been taking sibce 2008 n 2012. At C7-T1 showed some disc bulge, that is not what my significant issues are! She put cervicalradiculopathy M54.12 which is a disposablel type diagnosis. Second appt. 2019 she put no back pain, no joint pain no neck pain, same on 3rd appt.

2020 which was over phone cause of covid.We did not have topic of pain come up at all on any appts n as usual I write on previsit form my med diagnoses n that am coming for med refills.Never been lookin for drugs or a drug seeker.Treatments would want are not covered, experimental, and my conditions can never be fixed. I do not want to talk or think bout pain n never have.

They document like you are when you not say anything n not asked about"pain" or they lie n say you told them pain gone or better.Every year at least once I put one page note in records my conditions all symptons worsened over years n I do all my own ADL'S. I had been and on federal disability for skeletal spine l5s1, now for elderly with, and amerigroup medicaid medicare wanted me kicked off medical so here n there would get dirty med staff hired puppets for insurance company lieing n fudging my records.

Have heard similar stuff about filthy dirty corrupted type IME doctors n staff at workmans comp and disability places.Am suppossed to be going to medical staff for routine basic care not a disabilty IME and not a quack conartist dirty adversarial payed off unscrupulous one at that! The clinic am in these dirty misconduct med staff cause a disruption in!

This will be first time have to go back to a dirty slander libel ratsnitch pcp med staff as in past they were temporary or left before my next visit.Dr. as I said on this ripoff report also noted on my 4th n last visit with her that I said my symptoms started after a fall in my 20's next to her falsified*tenderness positive on malingering exam. She just said cervical spine exam *tenderness positive.

That is dingbat flake slander libel defamatory Baloney Stuff n I never fell on my neck in my 20's n my neck issues started in my 50's n my back 1st diagnoaed in my 40's.Last visit she put no musculoskeletal back no joint pain and the *positive tenderness cervical spine while editing all my skeletal spine conditions totally out including lieing and saying I told her I had osteopenia when have osteoporosis since 2015.

They also not give me previsit form like usually they do where I write my diagnoses down n reason coming on last visit July 20,2021 and past does not go away! Gaslighting she n Amerigroup medicaid medicare have all info.They apparently dismissed ALL my medical skeletal spine permanent degenerative significant diagnoses to make it appear I am a dingbat flake who never had any medical condtions my whole life that would prevent me from not having enough SSA retirement credits which do not have.

I had live with companions not married to n took care of their kids n elderly family many years n got no credits for.Also diagnosed 80's 90's with nerve damage sensory neuropathy feet legs from sacral spine injury accidents. They fluffed that off as well.Dr. C.M. n insurance twisted that into a dingbat flake neck injury.

All my hx is documented with significant objective evidence in past going back up to decades ago but this indigent type outfit n staff can get away with fudging records n patient ignored to try n amend or note they falsifying. Any other place would fire them but they never held accountable this type outfit.They also want to make like I am not credible so not have to do any tests in future or treatments if arises.

They have a lot of sociopaths and people attracted to abuse in these outfits.Want no part of this but cannot pay outta pocket to get out of this type outfit. I had one treatment, only thing ever done by these medicaid medicare or Amerigroup Amerivantage that was done on my eyes completed in 2022. With what the pcp sent me in was wonder eye doc did correct tests to find my eyes issue and correct it but she did.

These outfits will actually send you out to other docs to dismiss you as not credible nothing wrong.I have heard bout that.Anymore this type insurance does nothing now anymore have heard......I have to go back to this pcp n will get more gaslighting n more to shovel. Ignore, fabricate, edit deflect is Amerigroup medicaid medicare!!!!

Update December 2021. Had sent my December 22, 2020 HRA to my clinic and pcp doctor and Amerigroup. It is in my clinic saw. Received voice message October 19, 2021 from Amerigroup nurse case manager telling me to call back Amerigroup Amerivantage main number to complete annualHRA. No way! Also they apparently using faked impersonated October 2020 HRA STILL as not due until December 22, 2021.

I did not call back.Would never even do unless in writing. Went to doctor appt. July20,2021 at clinic. 2 FALSE FAKE exams were in my medical records that day that were NEVER done. Saw it in November 2021. I wrote details in in email enclosing.

 Update, am enclosing the letter I received from Amerigroup Medicare Medicaid dated October12, 2020 with contrary date on right side November13,2020 stating I completed my Health Risk Assessment when I never did, and telling me to log in online to view fake HRA plan for me. I received this in late November 2020.Also enclosing Health Risk Assessment form I received from Amerigroup with date on right side October7,2020, that I filled out and completed December22, 2020, as had 90 days to complete. Enclosed usps notice that Amerigroup received HRA form ready for pickup December29,2020,and usps notification that delivered to Amerigroup December31,2020 at 8:58a.m.

Enclosed letter I received very shortly afterwards from Amerigroup with same exact date October12,2020 to log into this fake impersonated Health Risk Assessment I never did to get their health plan they assessed for me by telling me to log in online to view according to my answers on impersonated HRA I never did.(THEIR answers from nameless staff). Same EXACT October12,2020 letter except contrary December29,2020 real date on side. Not legit outfit they do not acknowledge the obvious.All said my opinions. GollyMolly

Update Jan.5,2021.I sent certified letter To Amerigroup Amerivantage Medicaid Medicare dated Dec.22,2020 with enclosed completed Health Risk Assessment that I DID, and not their hiding out coward degenerate  nameless anonymous staff.All said on Rip Off Report my own opinions.Amerigroup signed for it Dec.31,2020. Got notified by email from post office. Then I got a letter in mail Jan.5,2021 identical copy of Oct.12,2020 letter! Obviously bogus.Same envelope presorted Covington, Ky. so no postmark. This letter had extra paper in it that has MOC POC P.O.Box 62509 Virginia Beach, Va; as a return address which is same address I mailed HRA to addressed to MOC HRA.

They could not have assesssed my HRA answers and made a Care Plan THAT  fast and over holidays, and with same Oct.12,2020 date as same exact letter. Baloney Sandwich.Obviously nameless,faceless,anonymous staff, resent same exact bogus letter again right after they received my legitimate Health Risk Assessment that I REALLY did. They do things so OBVIOUSLY. NO LOGIC to it. They can do anything they want they are anonymous and even if not are immune to any consequences or even INITIAL ACKNOWLEDGEMENT of any wrongdoing and they know it. Skunks. It is all obvious to anybody the illogicalness of it. This outfit you cannot get an issue or complaint such as this acknowledged or reasoned about with, so all can do is ignore it and document and put online.They obliterate stuff and keep way they want and respond like they are editing and fabricating a story to third parties.

Nobody takes complaints hardly at all or will do or can do anything about it, it will get fluffed off by any outside help sooner or later, they give up or are too afraid, or you would need to pay a lawyer.Staff from these outfits will snarkily tell you that too. Somebody is paying them to do some type of medical records fudging,and charging up for unneeded services also uh duh duh.Lowlife,gag slime.Barf Bag. Also harassment, trying to annoy and alarm. I am gonna put everything on blogs with paperwork period and not waste time thinking about other than that. Ignoring only thing can do.I do all my own ADLs, manage fine my chronic degenerative multi level lumbar lumbosacral and cervical spine issues by activity modifications, and normal regular HRA responses, they not want to use my real HRA uh duh uh duh.I go to doctor about 2 times a year for standard blood tests and a few standard type meds. They do not want to pay for that but they get paid off to fudge records and agenda to line their pockets. People with physical disabilities,elderly,indigent can get stuck in this skunk outfit, as Social Security makes everybody sign up for it.

An indigent clinic type place you can get better care and not this corruption but since you have medicare or medicaid mandatorily assigned, not elgible to go anywhere else unless out of pocket.I sent my doctor and clinic HRA and told them of issue. They of course get paid by this Amerigroup medicare medicaid which is rather icky.

My AmerigroupMedicareMedicaid Health Risk Assessment form dated October2,2020 sent from Amerigroup, is from,Early Intervention Case Management Team,which is just the general name of the outfit. I had one page not at hand when I initially wrote this report.Also it says after assessment care plan put on Amerigroup website to be viewed. So that is required.It says you can complete HRA over phone or by mail.I choose I will do only by mail.It says you can have somone else fill out form. I only will fill my own HRA form out and check that box.

It says you can choose not to have a casemanager and I will choose not to.The Sept.30,2020 phone conversation I had with Amerigroup primarily regarding updating insurance with a few health questions at very end, had NOTHING to do with Health Risk Assessment done once a year. Directions say to complete HRA form by 90 days and before start of 2021.The bogus Oct.12,2020 letter from, AmerigroupTexasMedicareMedicaid signed, Early Intervention Case Management Team, that was in a junk mail presortedfirstclassu.s.mail CovingtonKy. no postmark that I received Nov.21,2020, seems was concealing real date and backdated to obliterate my real HRA done by me that was to arrive dated sometime later after that date.Seems like bogus HRA would have fowl prank answers fabricated most likely as a criminal degenerate prank,harassment,abuse,taunting by sociopath abusers in staff and to higher billing.

All I said here on Ripoff Report are my own humble opinions.If caught Amerigroup may bluff error.I updated before this post but do not think went through so this is update Nov.25,2020. Initially thought may be an error, but the junk mail envelope, with no postmark,and no return address etc; details make me think not.

Author adding details about,Amerigroup,Anthem,TexasMedicare,Medicaid,AmerivantageDualCoordination.HMO D Snp.The letter said, Thank you for completing your health survey. We have reviewed the answers that you provided on the Health Risk Assessment, and have developed a plan of care based upon your responses. AmerigroupAnthemMedicareMedicaid knows that this would be interpreted as a form that was filled. However, me thinks that this may be a twist where they used my answers to health questions from end of very long phone conversation to Amerigroup on Sept.30,2020.

Think perhaps insurance records may give impression answers were from a form not from a phone call. The whole thing in my only humble opinion is a scam to bill for unnecessary, uneeded,and unrequested services.If using phone call info that was taped,the answers I gave were changed by them because HOW could they have said they had a Care Plan for me signed, Early Intervention Case Management Team. All said my only humble opinions in any of my Ripoff Report. My answers on phone were 100% normal. Let them put it on CD and send it to me with Care Plan with in writing how they say I answered. They SENT me a HRA form in mail not due till end of December 2020, and they sure know that they will get same kind of average normal responses, so they may have not wanted it in writing cause they know I will have a copy of it.The letter was sent in a prepaidfirstclasspostage junk mail envelope with no return address no postmark. They obviously wanted to identify it as me receiving it close to the Oct.12,2020 date that was on it instead of when they really sent it which was near Nov. 21,2020 when I received it.

I received no phone calls,no text messages,no voice mail,no letters about this. Just standard info mail.I have not called or had any contact with them at all through present today Nov.24,2020 since Sept.30,2020 phone call.I told them all correspondence by mail, phone,text,and not on internet on Sept.30,2020phone call like I have been doing all along, as was previously on Amerigroup with different type of insurance before that.So un legit, no contact and they tell me go log on website for info.They would have mailed it or called texted or voice mailed if legit.This is COERCION and SHOVING DOWN THROAT bogus Health Risk Assessment and Care Plan.

I will not log in to the website to look at this fake stuff or anything else on Amerigroup website especially after this scene. They can  SEND IT IN MAIL like they are supposed to.I will not participate in plan and no casemanagers.Everybody who is on Social Security is required to go to Medicare or Medicaid unless pay out of pocket full price.The only calls I got from Amerigroup was town hall meeting robo type calls on my voice mail numerous times.There was vmail with every call and when I got done with voicemail on my non internet phone,phone number showed in a popped up text.Do not know if that shows phone contact in their end other than for the town hall robo calls it came from.

Also besides IdentityTheft,False Impersonation, whether by form or obviosly changing phone call answers, this a form of criminal prank, harrassment,abuse,bullying,and taunting.These outfits are SLANDER LIBEL SNITCH RATS to SocialSecurity and third parties to line their cr$$k pockets.It attracts sociopaths who are abusers and like to abuse and dominate and nobody to take it out on so vulnerable patients fulfill their sport.
